11/3/2023 EnchantaVaughn Approved Behavior: QMHP-C began session by self-report from client. Client verbalized to QMHP-he struggles with communicating his feelings when angry on a daily basis. Mother reports when he fails to communicate he will last out on others with behaviors lasting for up to 5 minutes of screaming to the top of his lungs. Behavior: During second portion of the session, client was often tearful throughout the session and avoided eye contact. Client continues to demonstrate negative behaviors 3 to 4x's per week in moderate duration and frequencies lasting about 20 minutes. Intervention: QMHP provided Client with a supportive environment by setting up relaxing and calming objects in a circle around him so that he would feel good vibrations and less distractions while sharing information that was hard to process in a more unstructured, toxic environment. QMHP encouraged Client to take time to prioritize his needs in an effective manner. QMHP encouraged client to keep a daily journal to document his daily activities, goals, and occurrences. Client's medication compliance was also monitored. Intervention: QMHP engaged Client in a discussion regarding respectful communication with family members, and the counselor discussed with him the impact that anger can place on his health. Client was encouraged to verbalize his feelings of anger in a controlled manner. QMHP encouraged client to express his depression causes, as Client discussed his feelings of depression he was provided with support through head nodding, thumbs up, and active listening skills. QMHP assisted client with a plan for routine physical exercise to reduce his feelings of depression. Response: Client stated that he is capable of creating a daily log of his activities and will be responsible for taking his medication on a daily basis, so that he wont be so anxious and that he is able to have a better sleeping pattern so that he is more prepared for the day. Response: Client expresses his faults and the role he plays in the drama between his mom and dad. Client stated that he will try to get along with his family members better because he doesn’t like having conflict in his household every day.
